Paediatrics: what is the torsion of the hydatid of Morgagni?

Torsion of the hydatid of Morgagni causes sudden and intense pain in boys, especially between 3 and 11 years of age

It resolves with rest and anti-inflammatories.

The hydatid of Morgagni is an appendage located on the upper part of the testicle and represents an embryonic remnant

It may or may not have a pedicle.

It is found in approximately 90% of males.

Due to the presence of the pedicle, the hydatid of Morgagni may twist without a clearly identifiable reason.

It occurs most frequently in children between 3 and 11 years of age.

The main symptom is an intense and sudden pain in the testicle.

Redness and swelling of the scrotum is also present.

On palpation, a small swelling of soft consistency can be recognised in the upper part of the testicle.

Diagnosis is facilitated by transillumination, which is performed in a dimly lit room by placing a light source (a torch) behind the scrotum and which allows a small bluish spot to be seen (called a blue dot sign, present in about 1/3 of cases).

In certain doubtful cases, ultrasound is used.

Morgagni hydatid torsion must be distinguished from testicular torsion and epididymitis

Treatment of Morgagni’s hydatid consists of rest at home and pain-relieving and anti-inflammatory therapy.

Once the Morgagni hydatid torsion has resolved, the problem will not recur.
